What it includes:

  • Observation of mood, hygiene, and daily living.

  • After-visit summary provided to family outlining observations and recommendations.

  • Advocacy and collaboration with care staff.

Care Consultation for Long Term Care Communities

I help senior living communities improve communication, ease transitions, and reduce staff strain. Through flexible consulting, I can help identify care gaps, support complex coordination, and serve as a trusted family liaison - so your team can focus on exceptional resident care.

Care management provides the hands-on follow-through - coordinating care, monitoring progress, and supporting families 24/7 as needs shift or intensify, delivering peace of mind through attentive, expert oversight.

Care management services include, but are not limited to:
  • Ongoing collaboration with care staff, healthcare providers, and family members.

  • Regular review of care plans, behavior changes, and quality of life indicators.

  • Support during hospitalizations, discharges, or changes in condition.

  • Assistance with family communication and decision-making.

  • Coordination of outside services (therapy, home health, specialty care, etc.).

  • Objective, compassionate oversight to ensure consistent, high-quality care.

$220 per hour / $320 per hour nights and weekends

Ongoing care management services are offered through Transitions Care Management (TCM).
